Posted on Fri, 3 Feb 2017
Question: I have a pulse in my lower lip and chin area for 10 days. It comes and goes. My RDW - SD is 53. Is it the reason. Also I am overweight and drink alcohol. Although I have cut it down. I have also cut my smoking alot. And looking to quit. There is also some feeling in my back, left side. Just below ribs on the back. Also feel heavy in my throat sometimes. I do get anxious becoz of this. And when I get anxious usually at night, I feel like a big drop feeling in my chest. Please advice if I should go for some tests. Thanks in advance.

Brief Answer:
This looks to be symptoms of anxiety

Detailed Answer:
Hi,
This is Dr XXXX, cardiologist from Fortis Hospital.
The symptoms you are telling looks more like to be because of anxiety as you have feeling of pulse in your lip & back. They do not point towards any problem and occurs mostly when a person is anxious. Feeling heavy in throat & heavy in chest can be symptoms of heart disease as you have risk factors like smoking & abnormal lipid profile.
Coming to your reports, they are all normal except abnormal lipid profile. This currently do not need medication as per recent guideline. But you need to cut down alcohol & fatty food.
For cardiac evaluation, you should do a TMT or stress echo. For abnormal lipid profile, I'll recommend you for a ultrasound of abdomen as chronic alcohol use can harm the liver.
Also follow a health lifestyle, quit smoking.
Wishing you a good health.
Regards.

I have actually started running and I run normally. Don't get out of breath much. Although I will go for the tests mentioned. Also I read somewhere that deficiency of B12 can cause all these symptoms as well. Correct me if I am wrong. Also Thank you for your early response. I will make sure to drink occasionally and quit smoking.

Brief Answer:
Please see detailed report.

Detailed Answer:
Hi Mr XXXX,
Vit B12 deficiency generally causes tingling & numbness as Vit B12 forms our nerves. It won't cause such feeling of pulse.
Still we Indians are commonly deficient in Vit D & Vit B12, so their is no harm in getting them checked.
And it is very good to heart about your resolution to quit smoking & decrease alcohol intake.
Wishing you a very good health & a successful year.
Regards
