Question : I have the feeling like something is being 'twisted' under my breast bone. The pain radiates to my back and sometimes up to my shoulders. It has been coming on gradually for the past 4-5 months but been lasting now the last 24 hours or so. At times it will take my breath away and has woken me up at night. It does not usually happen after I have eaten. I don't think eating is a factor. I had my gallbladder removed 2 years ago so I know that is not it.

Detailed Answer:
Hi and thank you for asking on HCM!

I read carefully all your concerns and I can say as follows:
1.Your symptoms seems to be related with:
- heart issues
- GERD or other esophageal issues( tumors/esophagitis)
2. This is why to my opinion you need to be examined with:
- ECG plus heart ultrasound
- upper endoscopy
3.Meantime I would suggest to :
- use PPI'S like Prilosec OTC
- use two pillows when you lay down
- avoid alcohol and smoking.

Wish you fast recovery!
