What Causes Radiating Pain In Inner Side Of My Knee Cap?

Question : I'm 72 years old, very active, exercise almost every day, alternate weights and cardio with swimming daily and a 30 minute brisk walk each day. For no reason I can think of my left knee, especially on the inner side of the knee cap started having a radiating pain, dull but with occassional spikes like a pinched nerve. When it is bad after sitting a few minutes I get up and move around and it is better but will return if I stand very long. I have diagnosed spinal stenosis on the left side at L5,6 for which I have had three epidurals. This pain is similar but not quite the same as my sciatica from that. This pain radiates down the inside of my calf. I have been taking 800mg of Ibuprofen before bed which helps some but I take a long time to be able to go to sleep because of the pain. Any diagnosis and suggested treatment?

Thanks for asking.
I am Dr Prakash HM and I will be answering your query.
Based on your query, my opinion is as follows:

1. Could be osteoarthritis of knee.
2. As pain is found more during rest and reduced with mobility, it could be early osteoarthritis.
3. Will require X-ray of knee for confirmation.
4. You need to
a. reduce weight,
b. Wear good sports shoes (while moving around to avoid further damage, even indoors - good soft quality foot wear)…
c. Also knee joint support
d.along with muscle strengthening exercises around knee joint will be helpful.
e. Physiotherapy will help in reducing pain through heat application, or transcutaneous electrical nerve stimulation. Active and passive muscle training will be helpful.
f. Local topical painkiller sprays. If necessary, ibuprofen can be continued as necessary.

Further joint damage needs to be avoided.
