The pain in the neck radiating to the left shoulder with no sweating, palpitation and shortness of breath is likely to be due to a muscular pain. You can ask him to avoid using a pillow, do a hot compress and apply local analgesic gel in the area. If the pain is relieved then give him a mild pain killers additionally. In case the pain is increasing then do attend an ER for proper prescription medicines.
