Question: Hello, post op 11 months gastric sleeve, no problems. Pain in lower right side of stomach since wednesday that radiates to left back and shoulder

Based in history you shared, the few top most possibilities of pain of this nature in this region need to be ruled out first.

As radiation to back and left shoulder can be due to irritation of phrenic nerve. The top most relation to post gastric sleeve patient is, peritonitis. It is called as Kehr sign, in which pain of any accumulation of fluid or blood due to gastric leak below the diaphragm or in peritonial cavity will lead to pain in left shoulder. But that has to be accompanied by fever or abdominal tenderness or stiffness.

Gall stones can also present like this and are recognized complication of sleeve surgery.

You need to consult the gastroentrologist or gastric surgeon, and may ne asked for abdominal ultrasound which will help to reach some diagnosis.

At presentation, EKG may also be needed, to rule out inferior wall ischaemia which can present like this.

Serum lipase, amylase levels alongwith liver function tests may be needed. Complete blood count to look for any blood loss or increased WBC.

Management will all depend upon the diagnosis which is promptly needed in your case

Thank you... stomach is tender to touch, when pushing down on right side of stomach i feel the pain instantly going to my left shoulder, having nausea on and off but i just don't feel right. I may pay a visit to the e.r. if not better in a few hours, thanks again
