What Causes Random Bouts Of Recurring Diarrhea?

Question: Started having random bouts of diarrhea about 5 weeks ago... Comes and goes, ok for a day or two then upset for a day or two. Heathy BM in the am then followed by 1-2 diarrhea, then rumbles in my intestines for an hour then ok. But overall feeling low on energy. 51 wm 200 lbs 6'2" normally healthy and very active. No dietary changes.

I have gone through your query with diligence and would like you to know that I am here to help. I can understand your symptoms and how they can be both frustrating and inconvenient. But for any diagnosis, the right information needs to be provided to a doctor and for that I need your help sir.

I would like you to try and recall all the instances when you have experienced the urge to pass a bowel movement and if it comes immediately after having a meal containing specific food items. Let me list some of the most common food items that can irritate an individual with bowel abnormalities and let me know if you consume any of the following and have the symptoms you have mentioned above:
+Tea
+Caffeine
+Carbonated drinks
+Sugar-containing drinks
+Oily/fatty/fried food
+Dairy products
-Gluten containing products
+Popcorn
+Chips

I would also like you to write back to me with the a reply on which of the following symptoms you have:
+Increased urge to pass a bowel movement immediately after eating any triggering factors (above mentioned food items or anything else specific in your case)
-Abdominal bloating
-Abdominal pain (if yes, then what relieves it or what do you think causes it?)
-Pain around the left side of the rib cage
-Loose or broken stools
-Mucus in the stools
-Blood in stools
-Increased amount of flatulence (gas passed)
-Great relief experienced after having a bowel movement
-Past history of having diarrhea without the presence of a fever

Ok im very regular with morning bm, i do drink coffee but my normal diet has not changed in years. With that said, i have nothing that really "trigers" it. Aways happens in the morning. Lower back pain has been an issue for years but has been worse last few weeks, muscular and dics pain, moderate, heat and ice help with that. as i have a history with bulging discs. Mucus at the end of the first stool is a good sign that ill be back on the toilet in 30-45 minutes, then it can varry from very thin to water. Normaly a second diarreha, followed with hrs of rumbles, belching and flatulants, but settles down in the evening... No blood, no past history really, I have been trying to change some eating habits to experiment but nothing really changed theoverall feeling. I also would say i have been eating more food latley, with weight loss of 5 lbs. in 5 weeks and lack normal engery level dailey. No matter what i eat.

Hello once again sir.

If at all there are no triggers, then there is an underlying condition that is causing your symptoms. I would recommend a few investigations to find the exact cause, like:
-Colonoscopy with biopsy
-24 hour pH monitoring
-Ultrasound of the abdomen and pelvis
-Complete blood count (especially a check on ESR, hemoglobin, serum vitamin levels, etc.)
-Stool analysis
-Check stool for occult blood

The weight loss is slightly worrying sir, and blood can at time go unnoticed in the stools. The fact that you have experienced weight loss and and are feeling lethargic could be a sign of some bowel irritation which has resulted in inflammation. The bowel has certain functions other than just passing of the fecal content. It is responsible for the absorption of certain nutrients from the contents passing through the colon, and if at all these parts of the colon are abnormal/inflamed or not functioning normally these nutrients are lost.

The non-absorption of these nutrients can easily result in weakness and lethargy that can cause your presentation and weight loss. I am almost completely sure you have some underlying gastrointestinal condition that needs elaborate evaluation and treatment.

For now, you could also initiate a multi-vitamin supplement to help deal with your lethargy and poor energy levels. Please keep me updated sir, and I hope you understand why we need to investigate elaborately in order to find the cause. Only after that can we initiate treatment in a more accurate and rapid manner, without this any/all treatment will be blind treatment.
