Question : Hello. I just noticed an odd hand rash on my 4 year old daughters hands. It's on both, only on the tops (not palms). I've never seen anything like this on her before. Do you know what it is?

Also, it looks like she has no other symptoms except she has a temperature of about 99.5 degrees.

Hi...this looks like urticaria or angioedema.

If you observe keenly, there is not only rash and discolouration, but there is also a mild swelling in the joints and the shape of fingers appears slightly swollen overall.

99.5F cannot be deemed as fever in a 4 year of unless she is very uncomfortable with that temperature. (As you mentioned there are no other symptoms other than this, I assume there is no fever or pain or hoarse voice or pain the tummy or blood in stools and blood in urine or joint pains).

I have a few questions -
1. Did she play in the shrubs or garden recently?
2. Are there any bug bite marks on the body?

This typed of rash and mild swelling usually occurs as an allergic reaction, sometimes with fever and sometimes not. This is amenable to anti-allergic medications. If you have Benadryl at home you can administer a dose and see a paediatrician for prescription. If she develops any of the symptoms mentioned in the parenthesis above, she needs medical attention.

Yes, she was playing outside in the garden some. No bug bites that we know of.

I will get her on some Benadryl and see if that helps it. Thank you for your help.
