What Causes Rashes Under The Breasts, On The Stomach And Inner Thighs?
Question: I have so far seen two doctors regarding my rash which started under my breasts, then belly, outer armpits and inner thighs, then a few between my fingers. Also a scattering on my back. Doctor one thought I might have an allergic reaction and prescribed a three day dose of prednisone. I am currently on my day two of prednisone and have noticed some spots have seen to recover and only a few new itchy ones are occurring. My question is, as the older rash seems to be getting better, could it mean doctor one is right and it isn't scabies? Doctor two said scabies although I cannot see any burrows and they look very much like pimples some with whitehead. Doctor two wanted to start treatment for scabies although she did not perform a skin scrape however, I am currently breastfeeding an 8 month old baby and the pharmacist told me I cannot use the premytherin when breastfeeding or on such a young baby. Also I have read the rest of the household must also be treated at the same time though nobody else has shown any symptoms and it's already day 5 of rashes for me. What would you advise I do?

I have gone through your query. Your rash looks more like scabies. Even the distribution of rash also look like that. Allergic rash is the one which comes and goes. Scraping and microscopy is the definite way to identify as by naked eye it is difficult. You can stop breast feeding till you finish treatment. I would suggest permethrin application and ivermectin for taking internally.

I want to know if it is in fact scabies would prednisone help with clearing the older spots or do spots stay the same until scabies treatment? Secondly, I am aware I have to treat my household including 8 month old, 3.5 year old and husband. However, how does one treat an 8 month old who has a history of eczema? Can you please clarify. I am well versed with the treatment of school aged children and adults, but nobody is confidently telling me how to treat my baby who is beginning to show skin coloured spots and there is no point in just treating me without treating everyone else. Further to this, are skin scrapings done by a GP or dermatologist? I am in Australia and its months wait to see a dermatologist.

Usually scraping is done by dermatologist but even trained GP can do it. Usually scabies will spread to others also. So better to confirm it by doing scraping. Infants above 2 months can also be treated with Permethrin. In case of history of eczema you have to consult your dermatologist before treating. Prednisolone is an anti inflammatory. So it will be helpful for the lesions which were infected before.
But if it is secondarily infected with bacteria then it can worsen also. So I don't recommend prednisolone. Instead I would recommend antihistamines like cetrizine ( zyrtec ) once daily. You can also apply brilliant green or betadine on the older lesions.
