Question: I have pain every time I poop.and it's substantial pain..I have tried hemmeroid stuff. Doesn't seem to work very well...I have been trying to keep my poop soft with prune juice thinking that might help...it still hurts no matter how soft...

How long do you have this pain? Is there any trace of blood on stool surface?

Rectal pain during bowel movement is more characteristic for anal fissure than for hemorrhoids. Such pain is strong. Trace of blood can appear on stool surface.
Fissure is nothing more than a wound in rectal area. As this part is covered by mucosa, not skin, this gives stronger than typical wound pain.
Treatment depends on how long do you have it. If its present for weeks, no longer than few months, medicines should be tried first.
If you have this for years, surgery might be the only solution.

I assume that yours pain is there not that long. You need to know that to heal fissure, rectal muscle relaxation is the most important thing. To do that, Nitroglycerine cream, calcium channels blockers tablets (like Nitrendypine) or even Botulline injection should be tried.
All this medicines are on prescription only. And this diagnose needs to be confirmed by a proctologist. That is why I suggest you to visit one and have rectal examination performed.

yes, there is blood on the stools and also on the toilet tissue when I wipe. I have had the pain for about almost 2 months now. it first was here and there now it is with every bowel movement. also I wipe clean and even use those moist charmin mates. so I know I am clean. then the next time I go to the bathroom and just pee. I feel like something is there. I wipe my hind again and there is poop. I don't understand it like its leaking or something. sometimes its a lot and sometimes not.

If you feel something is there, it would be good to have it checked. Without rectal examination it is hard to give more advise. Please note that hemorrhoids and fissure are not the only options. Over the age of 50, the risk of colon cancer also rises.
So please start with proctologist or general surgeon consult.
