Question: In cool conditions any skin which is uncovered goes red and blotchy and very itchy or if I put my hands in cold water and recently it has gotten worse

You have identified the trigger for your symptoms which is exposure to cold. This condition fits cold urticaria. Blood counts and hepatitis profile should be obtained as some malignancies and hepatitis type can lead to such presentation. Raynauds should also be looked for especially if the fingers become whitish / purplish and as a result become painful due to inadequate circulation. Try to stay warm, use antihistamines and consult your doctor for alternative options such as doxepin or a tricyclic antidepressant which have shown some good symptom control in severe cases.
