Question: Hi Doctor,

I have been suffering from migraine since last 8 years, I have tried using sibelium 5MG(flunarizine)
for few days and SOS as suminat 25MG.tried Homepathy got relief. But from past 3-4 months I have been experiencing strange things in my head. some weird sensation in my ears, red lines in the eyes, severe neck pain (right side) radiating to back side of the shoulder. 4 months back I also got severe pain near my ear in the night, ( b'coz of the pain I could not sleep rest of the night) I realized that my pain is getting from upper right Jaw. I have consulted Dental doctor he said that it is due TMJ dysfunction. I have also consulted physician and he too checked and confirmed that nothing is wrong, and might be TMJ.From past 25 days I am getting severe headache(right side) including neck.As I was not feeling comfortable with the things which are experiencing in my ears, eyes and neck and head I have recently consulted ENT head and Neck surgeon, he examined said that nothing is wrong, it should be migraine, but to rule out he told me to take BRAIN and NECK MRI.Brain MRI was perfect and Neck MRI showed some mild spodylosis(I have attached the results of NECK MRI)after showing the results back to ENT surgeon he referred me to orthopedic. He too examined and said that for your age the results shown in NECK MRI should be OK and asked to check vitamin B12 and D tests done( as I was getting joint pains as well). Results are as below:

Results of Vitamin B12 : 284.00 pg/mL
Vitamin B12 (Cyanocobalamin)
Serum
Chemiluminescence Immunoassay

Vitamin D
High Performance Liquid Chromatography (HPLC)
Vitamin D, 25-Hydroxy :21.4 ng/mL
Serum
CMIA

And suggested some vitamin B12 and Vit D supplements and injections for 3 months.
and said that I will be alright nothing to worry.

Right now I am taking sibelium 5mg(in the night started again from week days) Rozavel -F 5mg( in the night from past 3 years( as my cholestoral level is more, doctor suggested me to take) and vitamin b12 and D suppliments just started from today.

Please advise Doctors..?

Brief Answer:
All symptoms are Migraine related.

Detailed Answer:
Welcome to HCM. I am Dr Anshul Varshney. I am here for your help.

Your all symptoms are due to Migraine only. Most of the times Migraine headaches are mis interpreted as cervical pain and such MRI's are performed.

You mostly need to get your Migraine in control.

You can carry on with the supplementation of Vitamin B12 and Vitamin D, but they are not the cause.

Migraine headaches are usually unilateral, associated with throbbing, vomiting sensations, irritability, intolerance to lights and sounds.

Once we are sure that the person has migraine, we start our patient on Migraine prophylaxis.

For Migraine prophylaxis most commonly used drugs like Flunarizine (Sibellium), Propanolol, Amitryptilline.

I personally prefer to start my patients on two drugs, mostly Sibellium and Amitryptilline.

Once started on prophylaxis, slowly your severity and frequency of attacks would reduce , after a few weeks of treatment you will have almost no attacks.

Life style modifications are also required.
Seep at least 8 hours in night, that too daily in a fixed time slot. Avoid sleeping in day time.Avoid alcohol specially beer, caffeine, lemon, cheese in diet.

And my Neck MRI shows Mild Spondylosis ... is that ok.? Or Do you suggest any neck collar..?

Brief Answer:
No need of neck collar.

Detailed Answer:
Dear Friend. These symptoms are of Migraine only. It's not essential that everyone will have the same symptoms at the same time.
As far as neck collar is concerned, there is absolutely no need of it.
Just do Cervical exercises. The findings in your MRI are very non specific. They don't represent anything serious.
If you have any further query, please ask me.
