Question: Hello Doctor,

I m 25 yrs male and had an uprotected sex a week ago.

I have already done vdrl,hiv and hsv test and all the results were negative.It was performed right after two days of time.

Aa of today,I have developed some red spot around my penis .Precisely, just beneath the tip of penis.

I have attached a picture for the same.

I am really worried for the same .
Please help me.

Welcome to HCM.

You had unprotected sex one week before and the reports came negative after 2 days which is not considered reliable as the window period for HIV is 15 days to 2 weeks.

As far as red spot around tip of penis is considered it might be due to some other infection because the symptoms of Acute Retroviral Syndrom also take at-least 10 to 15 days to develop.

As per my advice you need to go for repeat HIV testing after 15 days with 4th generation test to be completely sure about your status. Till that time just be relaxed and hope for the best.

Lastly, as your friend I will advice you to avoid unsafe sexual practices to stay away from the risk of HIV and other STDs.

Thank you for taking up my question.

I actually forgot to mention that i had unprotected oral sex.We didn't have any form of intercouse except for genital to genital touching.

My hair and skin is getting itchy and have developed mild sore throat as well.
Should i be concernced? I am getting anxious day by day

The risk.of tramsmission of HIV from oral sex is very less so don't worry.

The risk of getting HIV infection from oral sex is very less as compared to vaginal or anal sex so you need not to worry much.

Your advices have been very helpful to me by all means and i am also feeling very comfortable in talking to you, wish i could get your number.

There's this thing that i want to mention here is i had an unprotected oral sex at a massage parlour.
I was embarassed by the whole incident and since then i am getting very anxious about all these things.

Activities: She just gave me a blowjob wiped with tissue and i engaged in cunnilingus and some lips to lips kissing and genital to genital touching.

We did not have anyvaginal or anal sex .
However, while performing some cunnilingus i came across with some vaginal fluids and had spit that out.

I have narrated you the exact incident which took place on 27 Aug'15 .

Please tell me do i need to be worried.

I have followed your advice and got the test done after 15 days.I went for HIV 1&2 ELISA and it came non reactive.

However, as of today i have got some white color thing on my middle finger.Upon scratching it becomes red and i could also see some red dots.

Also i saw some black color lesion around my penis.

I have attached a picture for the same.

Please tell me do i have to again go for the test or not required.

You can go for 4th generation HIV test because ELISA only detects antibody.

First of all, your HIV 1 & 2 ELISA test is non-reactive after 15 days so it is quite relaxing for you but HIV ELISA detects the presence of antibodies only not antigen and the window period for HIV is 15 days to 3 months so to be completely sure about your status you can go for HIV 4th generation test as I had told earlier which detects the presence of both HIV antigen as well as antibody and it is more reliable.

As far as the lesions on your middle finger and penis are concerned they might be due to some other fungal or bacterial infection for which you can consult your local physician and take some treatment like avil or cetrizine or multivitamin as per the advice or go for some other investigations to isolate the cause. Don't worry much as the risk is very low in your case.

I have actually told them i want to undergo 4th generation hiv elisa test.

While the reports show hiv 1and 2 elisa.
I cannot see the word antigen on the reports.
So is it considered that they have included it under the sane head hiv 1&2 elisa?
