What Causes Red Spots With Pus Discharge Behind Ear Of A Child?

Question: Dear Doctor,

My Daughter is 4 year old and we live in XXXXXXX which is quite hot now.
Today night while sleeping my daughter had some irritation behind the ears , when i observed she had some red spots with pus coming out of it. We touched and it was sticky..
I am also sending the picture. Can you please let me know what it could and how to treat it.

Is it something serious or because of summers in XXXXXXX She goes to play school and comes in afternoon. Also she sweats a lot and her head is most of the times filled with sweat all over.

I have gone through your query and I have also reviewed the Image.

I can see a moist, red, patch in the fold behind ear.
This is seborrheic dermatitis which can present as itching, redness, scaling Or a moist patch with sticky discharge.
Scalp, retroauricular area, face etc can be affected with seborrheic dermatitis

Seborrheic dermatitis is a steroid responsive skin condition.
If I was the treating doctor I would suggest you to use a moderately potent topical steroid for her e.g fluticasone propionate 0.05% cream, twice daily, for a few days. This should make it fine within a few days.

I suggest that you take her to a dermatologist in your vicinity for a clinical examination and for the needful.
Topical steroids are prescription medications and should always be used under the supervision of a dermatologist.
Topical steroids should be used only for the recommended duration as advised by your dermatologist.
Self medications and overuse may cause side effects of steroid abuse.

Sure Doctor Thanks!! a lot. I will follow your instructions. But attaching one more better view of the image. May be this can help you identify the exact problem.

Also would like to know how does seborrheic dermatitis happens. I mean is it something serious and can spread or it is localized ? Also what precautions I should take for my daughter.

As compared to the earlier Image, in this Image the patch looks more wet and I can also see some yellowish/ honey colored crusting at the edges of the red, raw patch which suggests that the patch could also be due to a bacterial skin infection i.e Impetigo, rather than primarily due to seborrheic dermatitis.

'Impetigo' is a superficial bacterial infection which is characterized by raw areas/ erosions, oozing and honey colored crusting.

Impetigo/ bacterial infection at this site could be either primary due to hot weather and sweaty conditions which favor bacterial skin infections OR it could also be a superimposed bacterial infection on underlying seborrheic dermatitis.

Therefore after reviewing this Image I would rather suggest a slightly different line of treatment.
My treatment would first be directed towards resolving bacterial infection/ Impetigo with Oral antibiotic syrup e.g Syp. Augmentin for 5 days. Whether it is primary Impetigo Or superimposed bacterial skin on underlying seborrheic dermatitis, it does not matter at present.

In addition to Oral antibiotics you may also use a topical antibacterial cream e.g soframycin cream or mupirocin ointment, twice daily, at this site, rather than using a topical steroid cream which I had suggested in my previous reply.

Together these would dry up the infection.
