What Causes Redness On Penis After Having Intercourse?

Question: Hello my name is XXXXXX.
I have small redness on top of my penis. Skin is more red then penis skin. Inside redness looks like pale colour. I had it for last couple days.
I had sex with my fiance 15 days a go and after that it started showing. She doesn't have std but she did have a issue on her overy. Her overy was swallen and she was in pain. Now she is healed and everything is OK  but I stll have a red spot. Please help.

Hi, thanks for posting the concern in HCM.
You have mentioned that you have a lesion on your penis which has pale centre with red halo. This started after your last intercourse. Furthermore, your partner had a history of reproductive tract infection. You want to know what it is and what to do.

I think this is a sexually transmitted infection. Syphilis, chancroid, granuloma inguinale, psoriatic rash, fungal infection should be ruled out. Further evaluation is needed to find out the cause. For that some information will be needed along with some investigations. You can also attach an image of the lesion in the reports section.

Now, my advices are-
1. Maintain good local hygiene and avoid intercourse or masturbation till treatment completion.
2. Both you and your partner should visit nearest STI clinic for evaluation
3. Investigations - Complete blood count, urea, creatinine, HIV testing after proper counselling, VDRL, ANA, fasting sugar
Urine for Routine examination and culture and sensitivity testing
Urethral discharge smear microscopic examination
4. I need following information -
I. Do you have pain or itching in the lesion?
II. Do you have fever?
III. Does the Margin of the lesion is elevated with depressed centre
IV. Is there any scaly rash over the lesion?
V. Is there any ulcer?
VI. Do you have any foul smelling or abnormal urethral discharge?

Please write back with the information mentioned above and including all the investigation reports and if possible an image of the lesion.

Follow up: Dr. Rahul Tawde (1 hour later)
it has shown it self after 10 days of last intercourse.
I don't have any pain or itching
no fever
there is no scaly rash over lesion
its just there for last couple of days and I'm interested what us it and how can I remove it? I had a blood tests a few days a go and everything checks out. I'm very healthy. I'm attaching a photo of my issue. thank you

Hi, you have successfully attached three images of the lesion and thank you very much for the same.
You have also answered my queries. Now you want to know how to get rid of it.

From the image it looks like it is a ringworm infection of penis (glans) or Lichen planus of glans penis.

The margin of the lesion should be scraped and the tissue should be tested in KOH mount preparation.

A very similar appearance is found in case of Lichen planus of penis. Psoriasis can be safely excluded as there is no scaling. A suggestive point towards diagnosis of LP is the lesion is often asymptomatic like that of yours, while ringworm is associated with itching pretty often.

For confirmatory diagnosis of LP, biopsy is to be done. But, more often it is clinically diagnosed by a dermatologist.

In case it is diagnosed as LP topical corticosteroid ointment may help. In resistant cases, 0.1% tacrolimus ointment is prescribed and it may take upto 1 month to go off. In case it is diagnosed as a ringworm of penis, it is treated with antifungal cream and if needed oral Fluconazole.
