Brief Answer:
Continue with the same treatment as advised

Detailed Answer:
Hello XXXXXXX Welcome back.

I have reviewed the image and it looks better after just a day of treatment which is itself a proof that the initial diagnosis of candidal/ yeast jnfection is absolutely correct.
Redness is due to inflammation that accompanies yeast infection and it should resolve in a few days.
I suggest you to continue with the same treatment for a week.

Brief Answer:
Fungal infection is the likely cause for groin itching

Detailed Answer:
Hi.

Groin itching is commonly due to fungal infection/ tinea cruris. I don't think it has anything to do with recent protected sexual encounter.
Tinea cruris is specially common in summers and in humid weather conditions as there is lot of sweating from the groins.
Groin folds trap moisture and warmth & therefore provide an ideal environment for the fungus to grow.

I suggest that you first visit a dermatologist in your vicinity for a confirmatory diagnosis.

In my patients I usually treat tinea cruris with topical antifungal cream as well as Oral antifungal drugs e.g fluconazole tablet.

Cream needs to be applied twice daily whereas fluconazole tablet is usually once in a week dose

Duration of treatment with topical and oral antifungal, varies from 4-6 weeks.

One of the topical antifungals like clotrimazole 1% cream Or miconazole nitrate 2% cream can be used, twice daily for 4-6 weeks.
These active antifungal ingredients are available in various OTC creams as well as on prescription of a doctor.
Oral fluconazole tablet is a prescription product and should only be used after a doctors prescribes.

Along with topical and oral antifungals, In my patients wth tinea cruris I usually also suggest an OTC oral antihistamine e.g cetrizine 10 mg once daily for symptomatic relief from itching.

Hi sir thanks for the update.. I meed to follow up with some more facts/questions :-
1) these last two months i was in nairobi where weather was cool but used to get hot during afternoons and used to rain from time to time

2) could my condition be dur to irritation from the condom/friction? If this is not herpes is this just a coincidence that this has happened after five days ?

3) i am getting hiv test done after 2 weeks and then 4 weeks of incident i cant wait anxiety is too much. But in many sites says conclusive tests can only happen after 3/6 months?

Thanks a lot

Brief Answer:
P24 Antigen tests for HIV can be performed early i.e at 2 weeks and 4 weeks

Detailed Answer:
Hi.

This is not due to irritation/ dermatitis to condom. These lesions were classical of candidal infection.
Time line of appearance of lesions 5 days after sexual intercourse is not the only factor for diagnosis of genital herpes. The lesions don't look like genital herpes, therefore I ruled it out.
Herpes looks very different. There are sores and there is usually a lot of burning sensation and pain along with the sores.

Antibody based screening tests for HIV are conclusive at 3 months, however, Antigen based tests have a short window period and are conclusive at 2-4 weeks after risky sexual encounter.
P 24 Antigen becomes detectable at about 2-4 week period.
Therefore you may take a screening test for detection of P 24 Antigen at 2 weeks and again at 4 weeks and if these are negative, that rules out HIV.

Brief Answer:
Soothing lotion for symptomatic relief from itching.

Detailed Answer:
Hi.

Itching on arms, legs is not much of a concern.
You may use a soothing/ calming lotion on legs and arms e.g calamine lotion for symptomatic relief from itching.
Can also take a tablet of cetrizine 10 mg in evening if itching is troublesome.
Your urine frequency is absolutely within normalcy.
