Question: what causes repetitive night-time cramps in the top of the foot and sometimes in the calf. Starts generally on one side and sometimes continues on both sides.

Night time / nocturnal leg cramps are mostly innocuous and harmless and increase with age. Medicines like estrogen, naproxen, blood pressure meds, diuretics and cholesterol lowering meds or iron can also cause leg cramps.

Nutshell it is not to be worried about and should be treated symptomatically with pain killers. Meanwhile focus on strict glycemic control and good podiatric care for legs to prevent diabetic complications in legs.
