Question : my father aged 60 is a heart patient he had a stent put 12 years back.he is also an asthmatic and takes medicines for anxiety.he has been under stress for the last week.he also went for a golf tournament yesterday.wher he had outside food .today he complaint for severe pain on the right side out the stomach.he took devian for it.then at midnight he vomited and sweat profusely.he has been having bad burps and is still sweating in between. He is feeling anxious .His bp is low and his ecg was fine.what is the cause of all this and should we check.for his heart ?

Regarding your concern, I would like to explain that facing your father's clinical scenario, it is necessary to perform a differential diagnosis primarily between an ischemic cardiac disorder and digestive disorder (alimentary intoxication or even ulcer).

Though testing ECG has resulted normal, it is necessary to check also serial levels of cardiac enzymes (CK-MB, troponin) and a cardiac ultrasound to assure that an acute coronary syndrome is not evolving.

From the other hand, if the above suspicion (about a cardiac issue) is excluded, some symptomatic relief medications should be tried for digestive disorders.

Gastric antisecretors (like pantoprazole, omeprazole, etc) and antispastics (like buscopan,etc) and rehydrration could be of some relief in this regard.

Medical tests like an abdominal ultrasound, fibrogastroscopy and other lab tests (like bilirubin, ALT&AST, lipase, complete blood count, PCR, fecal ocult blood, etc) would be helpful in differentiating between several digestive disorders.

You need to immediately seek medical attention to promptly investigate the responsible cause of your father's complaints.

The gp came to see him he said looks like gastric only.he said sweating could be because of vomiting only.he has asked for some routine blood test like tlc dlc and liver function.but my father is complaining of ear pain in his left ear.are we on the right course?

Performing tlc, dlc and liver function tests are part of the differential diagnosis (among tests I prescribed you above). Investigating for a digestive disorder is important as gastrointestinal symptomatology is actually present.

Additional tests, like abdominal ultrasound, and even fibrogastroscopy (if digestive mucosa impairment is suspected) may be necessary.

Nevertheless, I insist in monitoring possible cardiac ischemia with repeated cardiac enzymes, as sometimes even neck or ear pain may be part of such an underlying symptomatology.

If evidence of digestive (or systemic) inflammation/inflection is present, then anti-inflammatory and antibiotics therapy would be appropriate.

Just follow his GP advices and also discuss with him about the additional recommended issues.

Doctor my father is better.he would like to know why alcohol doesn't suit him.he only intends to drink 1 drink maximum.he is an asthmatic and also takes depression and anxiety medicines.i am forwarding you his medicine chart please suggest the best combination for him

I am glad to hear your father feels better now. Sure that alcohol when taken frequently in increased amounts (>50 ml/day) may have adverse implications in several body organs like the liver, gastric mucosa, brain, heart, pancreas, prostate, etc. It may interfere with the metabolizing process of several drugs including depression and anxiety medicines (in the liver).
