Question : I have upper right abdominal pain. I've had it for about three to four months. It comes and goes, but I feel it everyday. I feel more pain when standing. I feel more pain the more I work out. I've noticed no pattern with what I eat. I've had an abdominal ultrasound - normal. I had a HIDA scan - normal. I'm an athlete (cyclist), 35 years old, 5'1", 115 lbs. Only health history issues is high cholesterol, which is hereditary.
I also have a pain in my left buttocks. It feels like I'm sitting on a rock. I don't know if they're related.

Brief Answer:
?? Suspecting a gastric abnormality

Detailed Answer:
Hello ma'am and welcome.

Thank you for writing to us.

I have gone though your query with diligence and would like you to know that I am here to help. Based on the provided information I am suspecting a few conditions like hyperacidity, peptic ulcer disease, etc. But these are just assumptions based on your current complaints, which is why I would recommend the following tests for confirmation or ruling out these conditions:

+24-hour pH monitoring
+Upper GI endoscopy
+Urea breath test
+Histopathoogy tests conducted with endoscopy

I would also like to ask a few questions and would greatly appreciate your kind response, thank you:

+Could you describe your eating habits to me like meal timings, gaps between meals, size of meals, etc.
+Smoking?
+Alcohol?
+Medications you are on?
+Stress levels
+Weight and height

Cycling and other forms of exercise can trigger an increase in acid production in the stomach, which is when the acid comes in contact with the mucosa (whether damaged or not), causing irritation. For this reason, I am strongly suspecting some abnormality with your gastric mucosa.

I hope you find my response both helpful and informative. I also look forward to hearing from you with your kind response in order to better understand your condition. Please also feel free to write to me about anything, I would be more than happy to help you.

Best wishes.

I am 5'1" and 115 lbs. I usually eat breakfast around 7:30 to 9:00. I eat lunch and 1:00 and dinner around 6:00. I eat small meals and pretty healthy. I don't really eat greasy foods. I don't smoke and I don't drink. I drink lots of water and no soda. I'm not very stressed and don't stress easily. Medication is the Nuvaring - February 2014 to present.
The pain doesn't seem to be in my stomach. It's in my right side. Laying on a heating pad helps it. It can hurt even when I am not riding my bike. Ibuprofen has helped reduce the pain to allow me to ride my bike. I only take it when I need to ride a few hours. If it's an hour or less, I don't take any.

Brief Answer:
A few suggestions and recommendations

Detailed Answer:
Hello once again ma'am.

Thank you so much for that information.

With the current new information I would also advice a HIDA scan, but as that has already been conducted we can almost completely rule out a hepatobiliary tract abnormality. Please also note that there can be various other causes for the typical pain you are experiencing which can be totally unrelated to the hepatobiliary tract; like for example irritable bowel syndrome, etc.

For this reason I would recommend you to undergo all the investigations mentioned in my previous response and also add a colonoscopy to that list. Please understand that your case may be a little complicated and so I would request you to have a little patience with the doctors who are helping to diagnose you.

I would also like to add that ibuprofen may be more potentially harmful than beneficial, at least until we can establish an accurate diagnosis ma'am, I hope you understand. I hope you find my response helpful ma'am. Please do not hesitate to write back to me for any further clarifications, I am always here to help.

Best wishes.
