What Causes Right-sided Pulsating Feeling Below Rib Cage?

Question: I have a pulsating feeling on, or below my right rib cage. Generally in my diaphragm or liver area. It seems most pronounced when I'm lying down, its noticeable when sitting, and I don't feel it when standing. It is not painful at all, just worrisome that it's been occurring for the past 3 days. Thank you.

I appreciate your concern

Normally, a person is unaware of his/hers heart beat. Unpleasant awareness of the heart beating is referred to as palpitations. It can be related to stress, anxiety or a panic attack especially if the heart rate is increased. A cardiac cause should be ruled out even though it doesn't appear to be the precipitating factor. An EKG and ultrasound abdomen pelvis can be advised to look for any rhythm / conduction abnormality or any anatomical cause responsible for the symptoms. These should be considered after a thorough physical exam looking for evidence of any abnormal pulsations or bruits upon auscultation of the liver. Treatment would depend upon the underlying cause however if anxiety or panic attack is suspected then propranolol and benzodiazepines can be advised in consult with your doctor.

Hello again.

Hemangioma and hepatoma are two possibilities that I would look for. The aorta would also be visible in the ultrasound and should be looked for aneurysmal abnormalities. If any anatomical abnormality or focus is detected , triphasic CT abdomen can be advised for confirmation. Most likely , the pulsations are subjective and can also be related to GERD( Reflux disease) with no serious pathology. I hope that is the outcome in your case which requires novel treatment with PPIs and pro kinetics.
