Posted on Tue, 16 Aug 2016
Question: I had a vasectomy about 5 years ago. No real problems other than now aware one side of scrotum far larger than other. Particularly obvious when scrotum is relaxed. I think it is swelling around the testical as opposed to swollen testical but not certain. I'm 54.

Brief Answer:
Ultrasound with color doppler.

Detailed Answer:
Hi.
Thanks for your your query, short and specific.
To recapitulate: Male/54 - vasectomy 5 years ago - one side of scrotum larger than other - wants to be certain.
My thoughts and advise:
Since vasectomy has been done, the chances of retrograde infection going into the testes or epididymes are nil.
Hence the cause is most probably hydrocele or if enlargement is due to testes it can be related to tumor or so.
Hence to be certain I would advise you to go for ultrasonography with color doppler examination - makes diagnosis almost certain. MRI too gives a good diagnosis if required but is costly and done only if there is doubt in ultrasound.

I hope this answer will give you appropriate diagnosis and a proper plan for further management.
