Question: My mother says that she can feel some kind of worm like creature under her skin, mostly around the top of her head. She is still very sound mentally but nobody else can see what she's talking about. She says the worm like creatures are leaving a sticky substance aroundvthecroots of her hair and mats it up. She has pulled a great deal of her hair out and has created a bald spot in t middle over hair. She says that starting yesterday that she has started feeling the "worms" have started moving into her arms and legs. I honestly think that she is ashamed of what the Dr will think if she goes to see them and she refuses to do so...any help with this issue is greatly appreciated.

Brief Answer:
Maybe psychotic disorder

Detailed Answer:
Hello, and thanks for your question.

What you are describing is consistent with a psychotic disorder. This may also be dementia, depending on how old your mother is. She needs to be seen by a psychiatrist immediately, to assess her for medications such as haloperidol or risperidone, which can improve her symptoms dramatically. If she refuses to see a doctor, I would call 911 to have her brought to an emergency room for an evaluation.

I can vouch for the fact that my mother isn't completely dilusional...this all started when she got lice from one of her grandchildren and of course she used medicated shampoo and combed all of the eggs out, however not too long after that she developed blisters on her head to the point that we started thinking maybe she has shingles and she even admits thatvit may have been attributed oovher scratching her head obsessively...so with that being said she now is certain that there are worm like creatures iin her head and she says they come out if her head and sew her hair to her scalp and they would push little eggs out of her head and they bust and sticky nasty stuff comes out...she constantly is asking people iif the can see what she's talking about nnd no one can seem to see what she says is there and s tends to get angry if you tell her its not there but I refuse to lie to my mother...I had thought about possibly using a placebo medication but I wanted to ask you if you think that would be a bad route to take...opinions and further questions are gladly accepted ...one last thing my mother is really sharp when it comes to most everything else, her memory isn't quite as good as she thinks but lol I can't tell her anything. I font think she is anywhere near dimentia and is fully capable of distinguishing real from fantasy. She does have quite a phobia of bugs and spiders though and when she sees one she either vacates the premises or says she feels like its in her so there may very well be some groundwork for delusional parasitism...please let me know what you think about the placebo idea and anymore input please. Thank you

Detailed Answer:
Giving your mother a placebo would be immoral. She needs to see a psychiatrist for delusions about bugs being under her skin, as this is clearly not true. She may need a medication such as haloperidol or risperidone, which can improve her symptoms dramatically. Again, if she refuses to see a doctor, I would call 911 to have her brought to an emergency room for an evaluation.
