Question : I have a diagnosis of ulcerative colitis. I have a doctor, but he's not available today. Had a sigmoidoscopy two days ago which confirmed I'm having a bad flare. Began 40mg of prednisone.

Since then I've had a dramatic increase in bleeding and pain. I cannot see the bottom of the toilet bowel after a bowel movement because of the dark blood filling the bowel. Blood clots the size of mice float on top of the water. I have a ripping pain. When I'm not having a bowel movement, the pain is more dull, a 6 out of 10. When having a bowel movement it is a 10. I sweat an dry heave from pain, and my heart races. I cannot relax. The blood that comes out now creates a deep, gripping pain, and often comes out into my underwear before I can make it to the bathroom. What should I do next? What should I ask for?

Hi Dear !! Thanks for query to HCM.
Read and reviewed the query facts in context to post-sigmoidoscopy griping pain with severe dark blood coming out of the bowel.
I assume you had a flexible sigmoidoscopy 2 days back. Please confirm this or say it was rigid sigmoidoscopy, as this would change the assessment of causal factors to some extent.

Brief up of Your health concerns -
and Current state of Your body-
Based on the facts submitted,Poor Vitals are perceived and indicated by the submissions made from You-as follows-
With sweats/dry heaving and tachycardia(heart racing)accompanied with the griping pain with dark blood coming out of the bowel, with incontinent bowel movements,Your general health seems to be compromised and stressed.
And hence as such, you need immediate medical check up and immediate treatment in the ER department.
Second Opinion from the ER -GI Endoscopist-Colono and Sigmoidoscopist is advisable in your case.

Replies to your Queries are as follows-
Q-1- What should I do next?
Ans-1-In absence of Your attending doctor, and thus in absence of any first hand information-as to what exactly happened during sigmoidoscopy, I would suggest you to Immediately shift yourself to ER for further treatment for the blood lost and for the griping pain with each of the dark blood bout during bowel movements.
Immediate investigations by-CT abdomen and USG and Standing X-ray abdomen,is needed to find out if any sigmoid puncture injury has occurred in the flared up Ulcerative Colitis Colon.

Q-2- What should I ask for?
Ans-2-In the given emergency situation-
Do's Suggested are-
Nil By mouth
Admission to ER and recoupment of Blood and fluid lost
Antibiotics ,
Antispasmodics,
Steroids just to control the body stress.
-and Investigation of the case- for puncture / or tear during sigmoidoscopy, so as to find out the cause of this sever post-sigmoidoscopy dark bleeding from the lower bowels.
-Consults and Second Opinion from-GI Colonoscopist/Endoscopist, from the ER department would resolve and fix the emergency condition and get it under immediate controls, as suggested above.

Next appointment-
Any time In case of any further emergency in the hospital /
or after 1-2 weeks,by which time you would come out and would be reviewed for further plan of treatment, if need be.

Hope this reply would help you to plan further care with help of the doctors attending on you.
