I understand your concerns & will try to help you in all possible ways.
Dizziness & passing out can be serious symptoms & should be evaluated in detail. It does not look to be occuring because of any drug effect but it can occur because of bp getting too low. It can also be because of several causes.
Brain causes - It can be because of ischemia of brain. You need to get a MRI brain & a EEG for it.
Heart causes - Decrease in heart rate to a very low level can cause such symptoms. For it we put a 24hr ECG monitoring called Holter Monitoring.
Abnormal electrolyte level - Hydrochlorothiazide can cause electrolyte imbalance. It generally causes low sodium. So please check his sodium levels.
So please get these tests done to rule out any serious things.
