Posted on Sat, 7 Jan 2017
Question: I am on the beginning stage of taking hydromorphone hcl 2mg medication prescribed by my pain management dr for back trouble. I have stage 4 CHF and sage 4 kidney failure.
Should I be taking a apioid medication with these conditions. I'm already dealing with lightheadedness
and dizziness with the lack of blood flow to my l head and this medication exacerbates this symptom
to the point I can't function.

Brief Answer:
could be worse.....

Detailed Answer:
NSAIDS lower blood flow to the kidneys and causes fluid issues in at least 1 in 4 of those given them if they have CHF. They can produce kidney failure either temporary and reversible when it is stopped or not temporary.
Tramadol may or may not work, it is longer acting and less strong than 2 mg deluded.
Narcotics do not change blood pressure. If lightheadedness is associated with low blood pressure then that can be found by checking blood pressure. If there is blood flow to the head being an issue then a carotid duplex has to be done to find out if this is occurring.
there can be mental effects of narcotics, they may get better in sort order or need a lower dose.

Thank you Dr for your opinion on this matter. I think it's best for me to avoid medications of this type as I'm already suffering with serious balance and dizziness and that is one of the main side effects of these drugs. I've tried Tramadol in the past and it was worse. There is an ongoing condition in my head that my neurologist is tracking. I failed to tell you also that I suffered a V-Tack in October that compounded my symptoms. I'll advise my pain management Dr.
Thank you again.

Brief Answer:
I think this makes good sense.

Detailed Answer:
This is one of those cases where I hate the term side effect. There was this one diabetic medicine that raised bad cholesterol and killed people, and a very similar one that lowered cholesterol and saved people. That was a side effect not related to those drugs action on diabetes.
But this muscle relaxer damps down the nerves and brain to be able to do what it does as a muscle relaxant. The same exact thing that makes it work will make you dizzy and fall. No getting around that!

It's kind of a straight down the middle effect.

And the same sort of effects by this type of drug will also affect the heart. It's not considered the best choice in someone with heart rhythm trouble.

You might consider a mineral supplement like magnesium which might help ...well... everything, the heart, the muscle spasms, maybe dizziness. It's really well documented for the heart rhythm and muscle spasms... one of the top choices for each.
