Intranasal saline spray and steam inhalation should be done.
Detailed Answer:
Thanks for your question on Healthcare Magic.
I can understand your concern.
I have gone through the prescription.
Nasal dryness is common after duolin Nebulization.
You can do following things to reduce and avoid dryness.
1. Intranasal saline spray and steam inhalation 4-5 times a day.
2. Ask patient to breath in through wide open mouth and rinse mouth after Nebulization.
3. If she is comfortable with inhaler then start duolin inhaler with spacer insted of duolin Nebulization.

                                    Brief Answer:
Oral bronchodilators should also be started.
Detailed Answer:
Thanks for your follow up question on Healthcare Magic.
I can understand your concern.
By your history and description, possibility of acute exacerbation is more.
So breathlessness on minimal exertion, fatiguability etc are symptoms of acute worsening.
Along with inhaled treatment, oral long acting bronchodilators like acebrophyllin sustained release (Abphyllin SR or Abflo SR) should also be started for better symptomatic relief.
