What Causes Severe Fatigue, Drowsiness And Tinnitus Post Intercourse?

Question: Hi,

I had protected sex with a sex worker 6 months back and she stayed with me for 2 days. She always felt tired and sleepy. I had asked why you are always sleepy and tired for that she just replied that she works 24*7 that's why. But I was not convinced also she had very weird smell coming from her body.

After 5-6 days, I started to feel extremely tired and very sleepy always. I also got tinnitus. I also started to get same smell from myself but that smell is completely gone now. But tiredness and sleepiness are not gone I always feel very week. After 10th weak I did STD panel test which were normal. But even after 6 months the tiredness and tinnitus was there so I did Thyroid test. My TSH was very high and my Thyroid Antibodies are also very high (both TG and TPO).

Is there and disease which I could have gotten from sex worker which had caused high thyroid antibodies? Also tinnitus? As before the incident I never felt tired and sleepy and the tiredness and sleepiness was sudden after the incident. What is the test and treatment option?

Welcome to HCM. I have gone through your questions and understand your concerns.

Your tiredness and sleepiness is related to hypothyroidism. Since your antibodies are high you are having Hashimoto's hypothyroidism.

Hashimoto's hypothyroidism (auto immune)is usually precipitated by some infections or stress etc. So probably you acquired some viral infection from the sex worker which has temporarily affected your immune system and increased the antibodies that made you hypothyroid.

The viral infection would have gone immediately within few days, but the antibodies produced will stay and have destroyed your thyroid gland and made it under functioning. Please consult a physician / endocrinologist and get started on thyroid medicines and your symptoms will return back to normal.

Hi,

Do I have to take thyroid medicine for lifetime? Why my blood test still shows such high levels of antibodies even after 6 months.

Is there any treatment to completely stop producing these antibodies and permanent cure to hashimoto's disease?

Can I take ultrasound of thyroid gland to evaluate the damage done to thyroid gland? Can my thyroid perform normal without medicine ever in my life?

Do hashimoto's patients get tinnitus also?

The answers to your questions

1. Around 90% of hashimoto's hypothyroidism is permanent. so you have to have life long medications in 90% of cases.

2. In hashimoto's hypothyroidism there is no treatment to decrease thyroid antibodies. This is because once hypothyroidism sets in even if you decrease the antibodies hypothyroidism won't improve. The treatment to decrease antibodies is immunosuppressants and steroid which are having higher side effects compared to thyroid medications

3. An ultrasound thyroid is useful to see the extend of thyroid damage

4. Chance of around 10% - more chance when your ultrasound is normal

5. Hypothyroidism can cause tinnitus

Hi,

Which is better to detect damage to thyroid glands due to antibodies Thyroid MRI or Ultrasound?

Also, I had 3 CT scans done in last 6 months and 1 more CT is to be done in next week So If I go with Thyroid MRI then do you think it is too much radiation exposure? Do you think doing 1 CT and 1 MRI/Ultrasound at the same day is too much exposure to radiation? How bad is these radiation exposures while performing CT/MRI/Ultrasound? How much risk I am in while doing these scans?

Hi,

Welcome back

1. For thyroid Ultrasound alone is needed as thyroid is a superficial structure well visulaised in ultrasound. MRI is a costly alternative.

2. There is no radiation risk with MRI or ultrasound. Ultrasound use sound waves and MRI use magnetic effect for scanning
CT scan causes significant radiation ( equivalent to almost 500 x rays) and is harmful.

3. You can combine CT scan and ultrasound/MRI on same day without any additional radiation risk.
