default
Posted on Mon, 18 Apr 2016
Question: Hi, I have been having groin pain, and when I have this pain I feel dizzy and at times feel like I want to pass out. The pain feels so bad. I have been to the doctor and even had an ultrasound done in the groin area. But they can't seem to figure out what could be causing this pain. Everything health wise checked out great with me. Is there anyone who might be able to shed some light on what the problem might be?

Brief Answer:
Please give detailed history.

Detailed Answer:
Hi.
Thanks for your query.

To recapitulate: Male/30 - very bad pain in the groin - feels dizzy and like want to pass out - Consulted Doctor - ultrasonography - every check up normal - could not figure out the cause...

Please give additional information:
- Since when have you got this problem?
- Can you please tell the exact nature of pain > burning, twisting, cutting, throbbing or so?
- For how much time does it last?
- Does it get referred to the testicle, inner leg, near umbilicus on the same side?
- Is the pain superficial under or on skin or deep within?
- Any associated urinary or bowel symptoms along with the pain?
- Any back pain or history of injury to the back / accident or so?
- Any other history which you may feel related to unrelated to the present problem, please give in details.
The answers to these will help me to assist you further.

Awaiting.

Hi,

Thank you for responding to me.

Here is the requested additional information

- I have been suffering with this problem for 4 years now

- The way the pain feels is like some one has grabbed your testicles and squeezed really hard, the pain is that similar feeling, only it's not in my testicles. It is like an aching feeling and a lot times it feels like that grabbing testicle type pain.

- The pain is constant and doesn't seem to ever go away. Most of the time it's not as intense, but the pain is always there. Then times it gets so intense and very painful, it makes me feel dizzy and nausea is bad and I feel like I want to pass out.

- The pain is on my right side, it's in my lower abdomen, in the space between my testicle and leg. Sometimes the pain will shoot to my side towards my back. But most of the time it's localized in that groin area.

- The pain feels deep within

- I have no associated urinary or bowel symptoms with the pain. I have been checked for urinary tract infection, kidney infection and hernia and don't have any

- I have no back pain or any injuries that I am aware of.

- I am just really worried about what could be causing this pain. I have been to so many doctors in the last 4 years and they have not been able to help me to find out why I am having this pain. It would be really great if I can finally get this pain to stop.

Thank you so much for your time and help in this matter, I truly appreciate it

Brief Answer:
As detailed.

Detailed Answer:
Hi.
Thanks for your feedback.
To recapitulate: 4 years - pain like squeezing of testicle - excruciating - constant - does not go away - sometimes exacerbated - with attendant symptoms - on rt - lower abd between testicle and leg - sometimes shoots to side *towards back -
deep within -

Few more questions please:
- Does the movement of the leg like walking or lifting the right leg high up increase the pain?
- Any other actions that can increase the pain?
- Any factors that can reduce the pain?

Since you have mentioned the area very properly, I can think of the following things:
- Femoral hernia
- Osteitis of the pubic ramus (the bone)or pubic symphysis.
- Prostatitis - best examined by per-rectal finger palpation examination.
-
A proper clinical examination, per-rectal examination and most importantly the combo-study in the form of MRI and CT scan considering these points can give the diagnosis.
Please discuss this with your Doctor and get it done, we may get the probable diagnosis, making the treatment possible.

So let us start with these and see the results.
