Question: Hi my 23 year old so who ise to smoke on and off for five years. he has a severe headache, body pain, congested chest, fever and cough with thick dark yellow stuff.

Since he smokes on and off, possibility of acute bronchitis is more.
It is common in smokers. It is mostly due to infection.
His symptoms like chest congestion, cough, yellow mucus, fever etc are suggestive of infection and bronchitis.
So first of all ask him to quit smoking as soon as possible.
Take him to pulmonologist and get done
1. Clinical examination of respiratory system.
2. Chest x ray
3. PFT.
Chest x ray is needed to rule lung infection. PFT is must for the diagnosis of bronchitis. It will also tell you about severity of the disease.
And treatment of bronchitis is based on severity only.
He will need broad spectrum antibiotic like levofloxacin or azithromycin. Inhaled bronchodilators (formoterol or salmeterol) with inhaled corticosteroid (budesonide or fluticasone) are also helpful.
So consult pulmonologist and discuss all these.
