What Causes Severe Headache In A Patient With High BP?

Question: i have a severe headache since 15 days which usually starts in the evening and usually takes two painkillers paracetamol and ibuprofen to calm down. but still the head feels heavy and when its too sharp i feel nauseous. i also had a very small bleed from left ear. i have hypertension but bp is normal 130/86. what can be possible reasons.

As for the ear bleed, it's not normal and can cause infection and severe vertigo. Bleeding means the ear drum may be affected and you should meet an ENT surgeon as soon as possible.

hi, thanks for you help.
About the headache it is like spread throughout the head like a throbbing one, i also feel involuntary twitching of muscles like eyebrows, thigh muscles . I have headaches before but never on a regular basis. I got my eyes checked six months ago so i dont think thats a problem.

And the bleed was not too much just a little and some clotted blood.
I had an MRI of the head i think 2 years ago and there were no abnormalities.

I still await some of the answers I requested.
However, I think I would think of migraine as a possible cause in view of the additional symptoms that you have suggested.
I would suggest a pain killer like naxdom or paracetamol. This will help in terminating the episode. In case the headache is frequent, prophylactic therapy can be suggested.

The ear bleed should be looked into, even if small volume.
