Question: I feel a very strong heaviness in my chest. What is this a sign of?

Greetings, madame! I am Dr. Meriton. Welcome to HCM. I understand your concern.

If this is a heaviness or tenderness kind of pain in the mid chest, which is lasting for more than 10 minutes, I have high suspicion that this is a pain generated from your coronary arteries - anginal pain. This requires a prompt evaluation in the emergency room. I strongly advice you that time is of essence, and you should go to the ER as soon as possible, to get an EKG done and troponin and CK-MB (heart enzymes in blood stream) measured. In the mean time, if your systolic blood pressure is higher than 110 mmHg, a tablet of nytroglicerine under your tongue, can help releive your pain. It also could mean nothing special, this pain, but it mandates prompt evaluation.

REPEAT:

I DO NOT HAVE PAIN... BUT A SEVERE CHEST HEAVINESS... THAT DOES NOT ENABLE ME TO GO TO SLEEP.

I HAVE NOT HAD ANY SLEEP FOR THE LAST FOUR NIGHT (SINCE LAST WEDNESDAY)...

This chest heaviness is something I have had for around a month already... but mild.

This time, it is severe... and, as I had mentioned, does NOT enable me to go to sleep! No pain...

What should I do?

As I stressed above, angina is characterized as a severe chest heaviness or tenderness, rather than pure pain. The follow-up information you provided, makes me suspect more for an acute ischemic problem of the heart. As I mentioned above, there is a slight chance this is nothing, and I hope this is the case. But the symptoms you provided, furthermore that this was a slight tenderness for a month and today is more severe, prompts immediate evaluation with an EKG and cadiac enzyme levels in blood stream. My strong recommendation is that you go to the nearest emergency room, as soon as possible. Of course, I only give recommendations to what my field expertise is. The choice is always yours.
