Brief Answer:
Clinical assessment, bile salt in blood & urine

Detailed Answer:
Hello,
Thanks for posting your query on HealthcareMagic.

Extreme itchiness can occur in a number of conditions.
Whether the itchiness generalized or is it localized?
Some of the important conditions are elevated bile salts and primary skin conditions.
Itchiness will be generalized in elevated bile salts and it will be localized in local skin conditions.
Thus, I suggest estimation of bile salts in serum and urine initially,
At the same time you need to consult a primary healthcare provider for clinical assessment of the site where itchiness is intense.

If you have more queries, I will be glad to answer.
