What Causes Severe Kidney Pain?

Question: My husband has severe pain in his left kidney area. He says it comes in waves. It makes him feel nauseated. He seems to feel a bit better when he is standing or walking. What should he do?

Hi XXXX Good morning. I am Dr Shareef answering your query.

Although your husband needs a complete physical examination before arriving at any conclusion, from your history it seems to me that he has got a ureteric colic in which the pain could come in waves. This is due possibly to a bstruction to the urine flow in the ureter most commonly by an impacted calculi (stone) in its course. This will also cause nausea along with severe colicky pain.

If I were his doctor, I would advise him for a urine routine/microscopic test, an ultrasound abdomen to confirm or rule out our diagnosis of ueteric calculi, and then refer him to the ER of the nearby hospital with the reports of the investigations. However, my primary aim would also be to relieve him of his acute pain, for which I would prescribe him an anti spasmodic drug along with an anti inflammatory like diclofenac sodium / Ibuprofen and a proton pump inhibitor with an anti emetic drug. He would be advised to take sufficient liquids as well.

Since he seems to be in acute pain, I suggest he visits his local physician / urgent care clinic / ER and discuss about the aforementioned tests. In the meanwhile, he can start ibuprofen twice daily.
