What Causes Severe Low Abdominal Pain And Pressure?

Question: My husband returned early from work this afternoon with severe low abdominal pain/pressure. He states that he wishes he could pass gas but can't. He is not experiencing fever and it doesn't seem to have a rebound effect if he puts pressure on the area of tenderness and quickly releases. I am suspecting appendicitis, but could it also be something else?

Hi.
Thanks for your query and an elucidate history about your husband.

To recapitulate: Male/50 - severe low abdominal pain/pressure - can not pass gas in spite he wishes so - no rebound - no fever - you are suspecting appendicitis - wants to know more...

The appendicitis pain is usually in the right lower abdomen, there may be associated nausea and vomiting and the feeling of passing gas is not the usual presentation.

Since the pain is in the lower abdomen and the feeling of passing of gas is there it is possible that he has some form of enteritis (small intestine).
Give him some antispasmodic medicine like dicyclomin, and see the effects.
It is possible that he could possibly pass gas and may get relieved.

If no results, take him to ER for clinical evaluation, examination, and few important investigations like X-ray of the abdomen in standing position, ultrasonography and relevant blood and urine investigations. By this time further symptoms may develop to give better idea about the possible diagnosis or he gets the relief.
