What Causes Severe Muscle And Joints Pain?

Question: March- April: Received 2 root canals. Still need a third for a very deep filling and exposed root.
June: Joint pain began. I was a runner so I thought it was associated to running. Orthopedist found my ASO titer to be 400. No treatment given. Fatigue sets in.
July: Ongoing fatigue becomes worse, white tonsil stones, low grade fever (99 degrees) Rx'ed 4 day Zpac. No improvement. Rx'ed amox..some improvement. ASO titer 379
August: Severe muscle pain begins, sore throat, white tonsil stones. ASO titer back up to 384. Rx'ed 10 day Penicillin. Symptoms improved (tonsil stones fell off)..unfortunately just finished course and the muscle pain is becoming very severe again. NSAIDS don't help. I've been to infectious disease, rheumatologist, and cardiologist. All other blood work is normal. What should I do now? I have left over Amox and Clindamycin. Should I take either since I felt better on antibiotics? Could this be related to my deep cavity? Dentists don't think so as there is no swelling in the mouth. Please help, I am so scared. I used to be so active and now can barely walk around the block!

Need to rule out Rheumatic joint disease, Rheumatoid arthritis

Hi there.

Pain in most muscles and joints if the body could have a wide differential diagnosis like Fibromyalgia, Rheumatoid arthritis, Osteoporosis with Osteoarthritis etc

If you are having fleeting joint pain then it could be Rheumatic joint disease but you don't fit in the age group commonly encountered with this disease. Your ALSO is high indicating possible Rheumatic disease due to bacterial infection.

This needs a multidisciplinary approach by Rheumatologist, Orthopaedic surgeon, Psychiatrist Neurologist.

I suggest you get treated for other arthritis like Anti CCP for Rheumatoid arthritis, ESR, CRP. Get a blood test after consulting with your Internist or Cardiologist to rule out Rheumatic heart disease which can be common. You can do Blood culture for Streptococcus species bacteria to confirm.

You can apply hot water bag to the painful joints and muscle pain. Eat a diet rich in calcium like eggs, cereals, pulses, chicken, lean meat, yoghurt. Drink plenty of water. Perform Yoga for stress relief. If you have difficulty in sleeping kindly consult with a Psychiatrist to help you achieve sound sleep.

Take combination Diclofenac with Acetaminophen tablet for relief of pain or Diclofenac with Thicolchicoside for relief of pain and muscle spasm.

Kindly provide more details as I have asked for the reports of the above mentioned investigations as well.

Thank you. I've already been to an orthopedist and there is no swelling of the joints. No signs of swelling in my blood test (c-reactive , rheumatoid factor, toxoplasma, cytomegaplasma, all negative) the only thing that remains high is ASO at 389 and I have 5.7 parvovirus b19 igg but I believe this is a common antibody. I went to an ENT today who scoped me. He found swollen tonsils and multiple white spots . He concluded the penicillin wasn't effective and started me on 875mg augmentin twice daily. He would also like me to take prednisone for 5 days . I am nervous to take the prednisone. Does this treatment seem appropriate ?

Hi there.

It is good decision to start Augmentin for the Follicular tonsillitis to treat your Strep infection.

Take the course of antibiotics for a week and the joint pains could resolve with this regime because the bacteria Strep is the cause for this which will divisive after the treatment.

Apply hot soaks. Take pain killers.
