What Causes Severe Nausea After A Meal?

Question: Hi. I frequently have severe nausea and pukish feeling at night after dinner. It is accompanied by uncontrollable shaking and trembling. I often have acidity and have to take antacids every night. I have undergone gastroscopy but the report was normal. I cannot sleep the whole night because I'm afraid I will vomit if I lie down. Please let me know what could be the cause and what would be the correct line of treatment. I have been having these symptoms since last 5 years.

Stress makes the symptoms worse and sometimes brings them on.

I have gone very carefully through your query. Your problem seems to be due to gastroesophageal reflux. Our gastrointestinal tract has a forward propulsive movement (peristalsis) that moves the food material forward as it is digested, absorbed and finally expelled as feces. If somehow this movement gets hampered, food along with the digestive juices would stagnate, stomach acid will reflux into the food pipe and on the other hand the bowel habits would not be proper. Since food is a trigger for secretion of stomach acids, the problem is aggravated on taking food. Stress too is an important trigger of stomach acids. Moreover, that is responsible for the pukish sensation that you get after the feeds.

In order to relieve yourself, you need to do the following:
* Avoid spicy food. Take small but frequent meals. Avoid long gaps between meals.
* Avoid alcohol intake (if it a part of your habit).
* Take Prilosec (omeprazole) once daily before breakfast.
* Take plenty of green leafy vegetables as well as other sources of dietary fibers. Take fiber supplements like isabgol/psyllium husk or senna. I would insist you to increase your dietary fiber intake.
* Regular morning walk and exercise to keep the bowel movements proper.
* If I had a patient like you I would perhaps have suggested Prodep 20 mg tablet once daily for at least 2 months (it may take time for the action to set in).
* Take curd or yogurt on daily basis. It contains beneficial bacteria and should help.
* Yoga and meditation should help.

Dear Dr. Saha,

I have already been following most of the things mentioned above:
1. Eat bland food
2. Take Omez or Pantoprazole
3. After food, take Gaviscon 10 - 15 ml
4. Ensure that diet contains leafy vegetables and fibre
5. Workout regularly
6. Consciously avoid stress all the time.

But still the symptoms return. I would really like to get better so that regular medication is not required. How can I try to achieve that?

Also, from your response, why is an antidepressant required as part of the treatment?

I understand that you have not been relieved in spite of the mentioned measures. That is why I suggested yoga and meditation as well. They are stress relievers and modulate the adaptability of the body. Prodeep (fluoxetine) apart from being an antidepressant is also a long term anxiolytic. We often have unresolved anxiety that we are not aware of consciously. This medicine helps by removing this stress which may otherwise serve as a trigger of stomach acids.
