Question : I went on March 9 to my dentist to have my upper teeth extracted for dentures. I guess my molars were deep and when they were extracted he perforated my sinuses. I was told not to cough or sneeze for a week and took amoxicillin for 7 days. With in 3 days I developed dry socket and had to return so that he could pack the socket with gauze soaked in clove oil. I went back 2 days later for a check up on that and he used surgical suction and he pulled out all the packing and the sutures that he'd placed by accident. But he sent me home with an RX for a medicated mouth rinse and within a day of that I was in severe pain again. This time I had dry socket in the original socket and now in another one right in front maybe from that suction?? So went back in and he cut some of my bone out of those sockets and packed the area again and stitched me back up...but to this day I am still in severe pain? Is this normal? When I take the dentures out to rinse they are always filled with a ton of saliva and it smells. This has been a complete nightmare for me and I never want to see the dentist again. Thank you, XXXXX

I have read your query and understand your concern . As far as severe pain and foul smell from extraction socket is concerned it can be due to Dry socket . Dry socket occurs when the blood clot at the site of extraction disloged . Exposure of underlying bone and nerves results in intense severe pain . As this is not normal so for this I will suggest you to take medicne like Amoxiclav with Metronidazole and Analgesic like Ketorolac DT by consulting with local doctor . Do warm saline rinses . Avoid forceful gargling . Usually placing zinc oxide eugenol and idoform dressing in the socket will make your pain varnish within few minutes . So good Consult Oral surgeon for clinical examination and dressing of extraction socket .

Thank you, yes I understand that I have dry socket but on Friday, March 18th I went back to the dentist and this time for two sockets...keeping in mind this was already done once and my teeth were extracted on March 9. On Friday since there were 2 sockets to fix this time we cut bone out of the socket is this normal? And could that be causing my pain? The dentist did pack the sockets with gauze soaked in clove oil and then stitched me back up so currently I cannot put zinc in the socket it is stitched up? That is why I am asking what could be wrong? Could this be from the dentist cutting out bone? Was it necessary? I see you have an antibiotic listed and a pain reliever listed for me to take above...I go back to get a check up as my dentures will need to be realigned they are very loose now. I am just not sure I trust the dentist after everything I have been going through?
And could my perforated sinuses be causing any of this? Or should that have healed on its own?
I just went to rinse and I did notice that the one socket is stitched up but the original socket is not and there is a yellowish reddish brownish drainage coming from that socket...where do I get zinc oxide eugenol and idoform dressing? AS I would like to do this now.

I have read your query and understand your concern . Your answers are detailed below :
1 . In dry socket condition irrigation and curretage of socket is done for fresh bleeding and so that new blot clot is formed in socket . After that zinc oxide eugenol pack is placed . So if your dentist has done Alveoloplasty (bone cutting) or curretage after extraction then there is nothing to worried so much .

2. You dentist has placed sutures so that blood clot will not disloged again from extraction socket .

3. As your denture is loose so for denture relinning you have to consult Prosthodontist . See XXXX what happened is when healing of socket takes place gums slightly changes its shapes that why your denture is loose.

4.As you are asking about sinus perforation see if there is perforation then you have symptoms like congestion or blood in phlegm . If you have these symptoms then I will suggest you to consult ENT surgeon for clinical examination and evaluation .

5. If you have yellowish brown drainage from one socket then consult Oral surgeon for Zinc oxide eugenol and idoform dressing . You cannot get eugenol pack you have to consult oral surgeon for dressing . Inmeantime you can do betadine rinses . Dont rinse forcefuly . Take medication for pain you will get relief.
