Question: my husband is almost 27 and he had no known help issues. but every now and then his lower abdomen hurts him bad and. swells out.about three finger tips below his belly button? what can cause that

It could be due to Divarication of Rectus muscles.

Detailed Answer:
Hi
As per your description, it looks like Divarication of Rectus muscle.Rectus muscle is present on either side of the belly button.When there is raised intra abdominal pressure, this recti gets separated and there will be bulging seen below belly button in some people.It is normal.
Another condition that has to be excluded is Hernia.
Pain in lower abdomen in the centre could be due to infection and associated inflammation of the urinary bladder(Cystitis).I prescribe Antibiotic like Levofloxacin 250mg twice daily for 5 days along with smooth muscle relaxant tablet containing Dicyclomine twice daily to my patients with similar complaints.
Get an Ultrasound abdomen next time when he gets pain.
