Brief Answer:
Precautions advised

Detailed Answer:
Hello sir, thanks for trusting health care magic.
This is common a problem and is seen in 4-5% of patients who undergo episiotomy.
It is because of healing tissue which may have subtle infection or due to poor tissue quality of the patient, contributing to pain.
In such a situation, I usually recommend my patients to take Tablet Lyser-D three times a day for 5 days and see the response.
It will help in healing and also decrease the pain significantly.
Also keep the area clean always, wipe it with clean tissue after using washroom everytime.
Ask her to avoid sitting in squatting position and keep her legs straight even during feeding the baby.
Take care

Brief Answer:
Wait for 2 days more

Detailed Answer:
Hello Sir, sorry for the delay in response as I was struck in an emergency surgery.
Yes she should jeep taking Zerodol and seftec CV as advised for 5 more days.
But in case there is a collection and pain doesn't subside with these drugs then she needs surgical exploration of the episiotomy site and release the swelling.
She should also take sitz bath viz sit in a tub full of lukewarm water at least twice a day.
Wait for a couple of days and then act accordingly.
Take care

Brief Answer:
better than that and safe to use

Detailed Answer:
Yes it is definitely better and should be take without a miss for 5 days at least.
Keeping the area clean is the mist important point.
But in my opinion it will need surgical exploration in the form of release of the swelling in the wound.
Hot sitz bath will give her a lot of relief.
All the best.

Madam surgical exploration is needed are it clear without that today dr Rx only trypsin and chymotrypsin and not antibiotic she said antibiotics is not needed
doctor
Answered by Dr. Manisha Jain (9 hours later)
Brief Answer:
Treatment needed

Detailed Answer:
Yes that is what I said that wait for two days on same treatment and in case pain and swelling persists then it will need surgical exploration and drainage of the involved area.
But in case pain decreases then only zerodal so will lead to complete resolution of the problem.
As far as antibiotics are concerned, you updated last day that doctor said there is persistent swelling and thus antibiotic still needed.
And in my opinion also if there is a lot pain and swelling antibiotic is a must as it is an infection prone area and needs lot of vigilance.
Take antibiotic and zerodal along with trypsin chymotrypsin as advised for 5 days at least.
