What Causes Severe Pain In Groin Area While On Josalid?

Question: Dear Doctor.

I previously write you about my on-going post hernia surgery issues.

Your last response was for me to see a doctor.

I saw my urologist two days ago. He physically examined me and pressed the area at the base of my penis. This is where i told him was very tender. He immediately said that my spermal cord was swollen and that it must be an infection. He prescribed Josalid for two weeks. He did not perform any other tests.

I have taken three Josalid tablets thus far and no change. (i know it too early to tell)

However, i just came back from jogging and i have a lot of pain around that area of surgery and in my testes. This suggests to me that this pain is not the result of an infection but it actually a mechanical one.

I really suspect that my sperm cord or other urinary tube is trapped somewhere, maybe either sewn or something.

What test can be done to be able to see and diagnose something like this?

My surgeon keeps telling me to wait and see, BUT i dont agree anymore since i have been having pain for over four months. This cannot be normal.

Good to hear from you again.
I had suspected cord inflammation earlier and had written to you that you might have Funiculitis. That is the term used for Spermatic cord inflammation.
It is unlikely that the cord would be trapped somewhere but sometimes it gets compressed or invested in Fibrosis around the mesh used to repair the hernia.
I suggest you stop the antibiotics and get a Urine and Semen culture done. This will clarify whether there is any active infection or not. It will also guide us towards what antibiotic would be best to combat the infection. Another approach would be to continue antibiotics for 10 to 15 days and then see. If the antiobiotic is effective then your problem should get relieved or decreased.
Investigations to see condition of cord would probably be an MRI but I do not think that it would be tied up in any suture. That is quite unlikely and have never seen it in my practice.
I suggest you wear tighter underwear so that the scrotum does not hang loose. This will help decrease the pain.
Rest as I have advised above. Do let me know the results if you get the investigations done.
Do not worry. I know it is a stressful time but it will get resolved.
