Question: At times I have severe short pains that occurs from the lower back and to my butt sometimes after a bowel movement.

I appreciate your concern

IBS is the most likely diagnosis based on the description of your symptoms. Other causes like external hemorrhoids, inflammatory bowel disease, anal fissure can also be associated with rectal / anal pain and discomfort after a bowel movement or with it. I prescribe Eucarbon in my patients which is a mild laxative with detoxifying properties. It contains the following

Veg. charcoal
Folium Sennae
Rhurab root
mint oil and fennel oil

Sucralfate should also be added to eucarbon. Mebeverine for prophylaxis and hyoscine butulbromide for acute relief of spasmodic pain can be used. High fiber diet e.g psyllium/ispaghula , stress reduction measurements like massage/hypnotherapy should also be implied. Try to avoid triggers that you can identify which worsen the pain e.g Alcohol, caffeine, excess red meat, smoking etc.

I'm also really gassy and at times I can't release it so I feel bloated and pressure builds up in my behind, but when gas is released it is such a big relief. It is embarrassing especially when I'm at work. I'd like to know how to resolve this issue. Thanks

Hello again.

Yes, this is one of the most common symptoms patients present with i.e relief of spastic pain upon passing flatus or after defecation. rifaximin is also an option that can be used especially with bloating and associated abdominal pain. There are many options for treating IBS. One or two medication are started based on the symptoms i.e whether the IBS is diarrhea predominant or constipation predominant or mixed etc and are then adjusted or added with based upon the patient response. Increased water intake, probiotics , fiber supplementation should be lifestyle measures that you incorporate into your daily routine.
