Posted on Tue, 6 Jan 2015
Question: Hello,
I am experiencing pain in my right hand near the thumb aera (inside the hand) since this summer. I went to see a physiothetapist and he thought it was Quervain syndrom and treated it for about a month. It has not made things any better. It still hurts and I ´m having a lot of difficulties writing, opening doors and doing day to day things. It really hurts!

Brief Answer:
Meeting a neurologist may be a good idea

Detailed Answer:
Hi
I am Dr Mittal

I would like to help you

Is there any quivering of the muscle?
Is there any weakness?
Is there any thinning of the muscle?
IS the pain extending?
Is there any extra pain when you bend your wrist?
Is there any tenderness? redness?
Is there any tingling numbness?

If any of these symptoms are present, it would warrant a visit to a neurologist. In that case, I would suggest testing a NCV and EMG to know if there is a nerve or muscle disorder that is setting in and needs evaluation, and thereafter treatment.

Also, I would suggest taking some rest. I prefer to let the muscle rest a bit. A minor period of inactivity for say 4-5 day may help your muscle rest and that may actually allow it to heal.

I hope to help you further if you provide the answers to the above questions.
Best of luck

Hello, the pain is sometimes extending to the arm. No redness nor thingling. No quevering either but tenderness yes. Doesn't hurt when I bend my wrist. Just when I do micro movement of the thumb. It also hurts when I don't do anything.

Brief Answer:
Sounds like myopathy

Detailed Answer:
Hi,

My impression of the condition, in view of that added information is that this could be a possible myopathy.
I would suggest meeting a neurologist. The tests needed are Creatine phosphokinase (CPK), Electromyography (EMG), and Nerve conduction velocity (NCV).
The treatment includes some medicines like steroids, so I would hesitate in suggesting treatment without confirmation of the disease
Best of luck

evaluation for myopathy vs neuropathy

Detailed Answer:
You can ask him for evaluation for myopathy vs neuropathy and testing of EMG, NCV and CPK.
I think he/she will not refuse.
Best of luck
