What Causes Severe Pain While Suffering From Pancreatitis?

Question: I have had chronic SIBO for about 9 years - am also lactose and wheat/gluten intolerant. I've used supplements and probiotics to lessen symptoms. Lately I developed abdominal soreness that radiates to upper left back (under shoulder blade). Lots of burping (not unusual for me) and alternating constipation/diarrhea. I had abdominal ultrasound about a year ago that found nothing unusual (just small hemangiomas on liver). I have an MRI and HBT / Lactose tolerance test scheduled and a colonoscopy. I am very concerned that there is something wrong with my pancreas. Can you advise?
I am quite anxious and feel heart fluttering/palpitations. Have had mood swings - not usual for me. I tend to be somewhat hypochondriacal, but am typically very healthy other than my digestive issues. SIBO testing indicated a very high methane issue / lesser hydrogen, but both are present...

Thanks for your query.
read and understood your history of SIBO, intolerance to wheat and lactose, probiotics.
Now developed soreness radiating to the upper left back under shoulder blade - burping - alternating constipation and diarrhea - planned for investigations - 'anxiety, palpitations, mood swings' (needs thyroid function tests) - and all.

I would advise you the following:
Thyroid function tests.
Contrast enhanced CT scan of the abdomen is still a better investigation.
Amylase, lipase, sugar in blood
Stool tests on three different occasions.

Your concern about Pancreas:
With all the symptoms you have, it is possible that the functions of the pancreas may have been altered.
The pancreas gives the pain exactly at the back portion where it is lying, that is in line with just above the umbilicus in the center of the back, is severe,
It will be well examined on MRI and CT .

Let us wait for the reports and see what else can be done.

Could pain from pancreas be intermittent, or would it be more constant?

Brief Answer:
MRCP will be diagnostic

Detailed Answer:
The pain can be intermittent in Chronic (old case) of pancreatitis.
The pain from acute case is constant.

Anyways, the MRI, preferably MRCP (Magnetic Resonance Cholangiopancreatography) will give us the proper diagnosis.
