Question : Last night my 69 year old husband went to ER with severe ride side pain, upper quadrant. Rated pain at 9-10 on scale of 1-10. He could not lay down, nor sit. They did a cardiogram and CT scan was performed with negative results. Following morphine for pain, they sent him home. Tonight the pain is beginning again, following eating. He indicates feeling pain, bloating. Should her return to E.R.?

Brief Answer:
Please have emergency consultation with gastroenterologist if possible

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. The abdominal pain in case of your husband appears to have a gastrointestinal origin as it has become severe after taking meals.

2. It will help to return back to the ER and have an emergency consultation with a gastroenterologist. Your husband might require medications to treat the primary cause of pain. He also requires investigations like upper gastrointestinal endoscopy to evaluate the lumen of esophagus, stomach and duodenum for any conditions that might be causing the intense pain and discomfort. The CT scan does not show detailed information on the lumen of esophagus and stomach and any small erosions or ulcers need to be looked for.

3. If an emergency gastroenterologist is not available then please arrange for an early appointment with a gastroenterologist in the next few days.

4. Please ask your husband to take smaller meals at frequent intervals and drink adequate amounts of water and fluids. He should not skip meals.

Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.
Do write back if you have any doubts.

Regards,
Dr.Vivek
