Brief Answer:
Following answer to your query.

Detailed Answer:
Hi and thank you for asking!
I read your query and understood your concerns.

Your symptoms (especially superficial epigastric pain that spread on the back) once that cardiac problems are excluded (with ecg and other cardiac tests ) may be related to:
- GERD ( acid reflux disease )
- Gallbladder/pancreas issues
- Musculoskeletal problems
- pulmonary problems.

If you were my patient I would recommend to do the following examinations to find out what is causing your symptoms :
- blood work including liver and pancreas enzymes,
- chest x ray (for evaluation of lungs)
- abdominal ultrasound (to exclude gallbladder and pancreas issues )
- upper endoscopy (to exclude GERD )

If all examinations will be normal than your symptoms are related to musculoskeletal problems.

Hope this was of help!
Wish you health!
Dr. Klerida
