Brief Answer:
May be related to the urinary system.

Detailed Answer:
Hi.
Thanks for your query.
Read and understood your problems.
To recapitulate: 55 / Female - sharp pain in the lower left of abdomen close to the groin - since yesterday morning - feeling heavy - whole abdomen is swollen - could not urinate in the beginning - spent the whole night in the bathroom.

The most probable causes are:
- Stone in the urinary bladder and / or in the left lower ureter causing all the symptoms you have mentioned.
- Cyst / Mass in this area causing the symptoms you have.
- Severe urinary tract infection.

The abdominal distension can be secondary to the urinary problem ans occurs as a reflex mechanism.

I would advise you the following:
- Send the sample of urine for routine, microscopy, culture and sensitivity examination before the start of antibiotics.
- Tests of blood to see for CBC, thyroid and kidney functions.
- X-ray of the abdomen for KUB (Kidney ureter bladder).
- Ultrasonography of hte whole abdomen
- IVP /CT scan of the abdomen if required.
- Antibiotic and supportive treatment

All these tests done under the guidance of the Doctor, preferably an Urologist will give you a proper diagnosis and a plan for a proper curative treatment.

I hope this answer helps you to get a proper diagnosis, and get a proper further treatment. Please feel free to ask for more if you need to or if you feel that there is a gap of communication.
