Brief Answer:
Pinched nerve in neck.

Detailed Answer:
Hello and thanks for using HCM.

I have read your question and understand your concerns.

Symptoms you describe are most likely caused by a pinched nerve mostly at neck.

Conditions that may cause such issues include degerative discs disease, herniated discs disease, arthritis and spondylolysthesis of the cervical spine.

A consultation with a Spine Specialist for a thorough clinical examination and imaging studies of cervical spine (MRI scan preferably) are necessary to get a correct diagnosis. Please discuss this with your doctor.

Hope you found the answer helpful.
Let me know if I can assist you further.

Kind regards.
