Hi Dear !! Thanks for your query to HCM.
Based on facts of your query,You seem to have IBS with spastic sharp pains sometimes, else seems to be dull aching pain on left side below and behind lower ribs.
You seem to have GERD with Constipation of the IBS.
The internal burning sensation with radiation to hip, seem to be from obstructed sigmoid with proctitis? or pelvic inflammation surrounding pelvic organs.
I would suggest you to Consult Surgeon and do proctoscopic study, which would fix the cause of IBS- which could be colitis with proctitis with -? Fissure in Ano ? or could be from some obstructing mass ? or could be from Diverticulitis.
Consult GI endoscopist who would do colonoscopy to fix the cause of your left lower dull pain.
You may need to do CT Contrast study of the Colon-with Barium enema , to find out the exact cause of this left lower abdominal pain.

Remedy-
I would suggest you to start-Tab-Bentyl
Tab Ciplox 500 mgm x 2 times a day
Tab Metronidazole- 400 mg x 3 times a day x 10 days time
Cap sporalc
Bland diet would resolve the issue in 2 weeks time.
If not Consult Surgeon /Physician who would investigate and treat it accordingly.
