                                            Question : I am a 46 or old man. Non drinker, use tobacco but otherwise healthy.  Every once in awhile I get a sharp stabbing  pain on the inside of my anus seems to be more on the side. I can't get the model to show the actual spot

I have gone through your query. Your problem can be due to anal fissure. A local examination by a general surgeon will be helpful to rule out this. Proctoscopy will be required in doubtful case.
Sits bath will be helpful for this. Use warm water to wash and soak the area after bowel movements. You can also apply diltiazem cream locally. Avoid constipation by taking stool softeners if you having it. It might take 2 to 4 weeks to heal if the anal sphincter get enough relaxation. If no improvement then surgical treatment can be considered.
