Brief Answer:
Do not give top feeds. Exclusive breast feeding is the best solution.

Detailed Answer:
Thanks for asking on HealthcareMagic.

Let me begin by congratulating you on having a baby. I have gone carefully through the details provided by you. I feel that things will be fine with just a bit of your awareness.

You need not worry about the sleeping patterns of a baby. It can be erratic. A baby may sleep 20 hours a day or may remain awake entirely during the night and yet sleep on and off during the day. Sleep need not be deep. Do not worry as the child will get the sleep it requires. It is not unusual for a child to cry for feeds immediately after passing urine.

Breast feeding is the best possible food for a baby and there can be no alternatives for the same. Give the child breast feeding at 2 hourly intervals or as demanded by the baby. Feed from each breast using one only during one episode of feeding and alternate sides for the next feed. Do not continue to feed the baby for more than 20 mins as extending the time will not increase the amount of breast milk available. Do not give formula feeds. If the number of episodes of passing urine would be more than 5 times over a period of 24 hours, you can assume the feeding to be adequate. If the child is given top feeds, its hunger would reduce and so the child will suck less. The amount of breast milk depends upon the demand of the baby which is manifested through suck. So, if top feeds are given, production of breast milk would automatically come down. If the child needs, amount of breast milk will automatically increase. Hence, simply do not go for top feeds. Formula feeds are difficult to digest and hence may cause a child to vomit. Unless you give top feeds and overfeed the baby, the child is not likely to vomit.

If the child is sucking, it has its voice box closed so that food does not go into the airways, and hence it cannot cry while sucking.

Hope that clarifies your queries.
