Have you had recurrent low back ache in the past? Do you have leaking of urine? If yes it is better to get fresh evaluation.
Else I think you should prefer an Ibuprofen tablet if the gel that you applied does not help. Along with that you should take a hot shower after applying the gel. Complete rest for one day.
